摘要:目的比较3种荧光定量聚合酶链反应(FQ-PCR)试剂检测痰标本中结核杆菌的灵敏度和特异性,探讨FQ-PCR在国境口岸快速检测结核病人的价值。方法 对129例疑似肺结核患者的痰标本分别采用FQ-PCR试剂法、痰涂片抗酸染色法、改良罗氏培养法检测结核分枝杆菌;以培养法为金标准比较艾康、匹基和博奥3种FQ-PCR试剂的特异性和灵敏度。结果 艾康检测疑似肺结核病病例、涂片镜检阳性和涂片镜检阴性者的灵敏度为90.12%、93.55%、78.95%,特异度为95.65%、100%、95.65%;匹基检测疑似肺结核病病例、涂片镜检阳性和涂片镜检阴性者的灵敏度为87.50%、95.16%、61.11%,特异度为100%、100%、100%;博奥检测疑似肺结核病病例、涂片镜检阳性和涂片镜检阴性者的灵敏度为86.59%、95.24%、57.89%,特异度为100%、100%、100%。结论 FQ-PCR是一种简便快速方法,对于涂片镜检阳性结核病病例的检测具有很高的敏感性和特异性,对于涂片镜检阴性结核病病例检测有很高的特异性,适于国境口岸出入境人员中结核病病例的快速检测。

Comparison of effects on three kits offluorescence quantitative PCR examining suspected caseswith tuberculosis at frontier ports

Abstract: Objective To explore the value of fluorescence quantitative PCR examiningtuberculosis at frontier ports through comparing the sensitivityand specificity of three kits for detecting mycobacteriumtuberculosis in sputum specimens. Methods Thespecimens of sputum from 129 cases with suspected tuberculosispatients were detected by three fluorescence quantitative PCRkits(AIKANG,PIJI,BOAO), acid - fast stain, culture with LowensteinJensen (L - J)medium, respectively, the sensitivity and specificityof three fluorescence quantitative PCR kits were acquired comparingculture results. Results For detecting suspectedtuberculosis and smear positive cases and smear negative cases, thesensitivity and specificity of AIKAN were90.12%,93.55%,78.95% and 95.65%,100%,95.65%, respectively. Thesensitivity and specificity of PIJI were 87.50%, 95.16%,61.11% and100%,100%,100%, respectively. The sensitivity and specificity ofBOAO were 6.59%、95.24%、57.89% and100%、100%、100%,respectively. Conclusion Fluorescence quantitative PCR, which is a rapid and sample method,own high specificity and sensitivity for smear positive cases andhigh specificity for smear negative cases, It is a good tool forexamining tuberculosis at frontier ports.
