2004—2011年上海口岸国际旅行者性传播疾病分析

    摘要：目的 评估上海口岸出入境旅行者中性传播疾病的流行趋势及危险因素。方法回顾性分析2004—2011年上海口岸国际旅行者中性传播疾病（艾滋病和梅毒）监测结果，对感染者进行流行病学调查，分析人群特征及危险因素。结果共调查国际旅行者523731人次，检出性传播疾病感染者613例。艾滋病病毒（HIV）感染平均检出率为27.30/10万，大部分为境外输入性病例。梅毒平均检出率为93.94/10万，超过半数为本地出境人员。检出率呈上升趋势。感染者中以男性20至49岁性活跃人群为主，≤19岁女性和≥50岁男性所占比例近年有所增加，平均年龄有增高趋势。性接触尤其是与商业性工作者及男性同性接触仍是主要传播途径。共发现22例HIV/梅毒双重感染病例。结论上海口岸出入境国际旅行者中性传播疾病感染率有所上升，在旅行医学工作中应该重视采取提倡安全性行为，筛查高危人群及宣传教育等干预措施。

    Analysis on sexually transmitted diseaseamong

    international travelers at Shanghai port

    Abstract:   Objective  To assess the trends and risk factors of sexually transmitteddisease (STD) among entry-exit international travelers at Shanghaiport.  Methods   A retrospective review of STD (HIVand syphilis) surveillance results among international travelersduring 2004 to 2011 was conducted. Patients diagnosed with STD wereinterviewed to analyze the characteristics and risk factors of STDinfection.   Results   A total of 523 731international travelers were enrolled. Among participants, 613 STDcases were identified. The average detection rate of HIV infectionwas 27.30 per 100 000. Most of them were travelers from overseas.The detection rate of syphilis was 93.94 per 100 000. More thanhalf of them were outgoing travelers. Assessment of annualdetection rate revealed an increasing trend. The predominantcharacteristics were male gender, sexually active age (20~49).There was an increasing trend of teenager girl and old age maleinfection. Casual sexual activities and men who have sex with men(MSM) remained the main route of STD transmission. Twenty-two casesof HIV/syphilis co-infection were found. Conclusion   There has been an increasing trend of STDinfection among international travelers in Shanghai. Promotingsafer sex, screening among high-risk groups and education areessential in the healthcare of travelers.
