抗结核菌糖脂抗原抗体及抗阿拉伯甘露糖抗原抗体并联检测在口岸肺结核病快速诊断中的应用

摘要 〔目的〕研究抗结核菌糖脂抗原(TBGL)抗体及抗阿拉伯甘露糖抗原(LAM)抗体并联检测在结核病血清学诊断中的价值。〔方法〕采集结核病患者及健康对照者血清,用酶联免疫吸附试验(ELISA)检测抗TBGL抗体,用胶体金法检测抗LAM抗体。〔结果〕91例结核患者TBGL抗体、LAM抗体总阳性率均为62.6%,其中活动性肺结核病人的TBGL阳性率为65.3%,LAM阳性率为69.4%;非活动性结核病人TBGL阳性率为59.5%,LAM阳性率为54.8%,两组间差异不明显。肺结核病人两抗体并联检测阳性率为89.0%,明显高于单独抗体检测阳性率(P< 0.05),特异性达85.7%。〔结论〕抗TBGL、LAM抗体并联检测敏感性、特异性高,方法快速简便,对口岸结核病预防控制有较高的实用价值。

[Abstract] Objective To study the value of tuberculosis glycolipid antigens (TBGL)antibody and lypoarabinomannan polysaccharide antigen (LAM)antibody in the serological diagnosis of tuberculosis insignificance. Method The serum samples werecollected from TB patients and healthy controls. Anti-TBGL antibodywas measured by enzyme-linked immunosorbent assay (ELISA), andanti-LAM antibody was detected by colloidal gold test. Result The positive rates of the TBGL antibody and LAMantibody detected from 91TB patients both were 62.6%. In thepatients with active pulmonary tuberculosis, the positive rate oftwo antibodies were 65.3 % (TBGL), 69.4% (LAM) respectively; and inthe patients with non-active TB, the positive rate was 59.5%(TBGL), 54.8% (LAM), respectively. But there was no significantdifference between the two groups. The positive rate oftuberculosis patients through the parallel combined use of testswith two separate antibodies were 89.0%, significantly higher thanthose detected by single antibody test (P < 0.05), and thespecificity of combined tests was 85.7%. Conclusion Thisstudy determines that the parallel combined detection of anti-TBGL,LAM antibody is a rapid, simple and high specific method, whichmaximizes the sensitivity and of TB serological diagnosis. Thismethod has a higher practical value for tuberculosis control atport.
